We recommend wearing comfortable, casual clothing to make your first visit as smooth as possible. While it doesn’t have to be athletic wear, choosing something easy to move around in helps.
We encourage you to complete the necessary paperwork before your visit to expedite your check-in process. However, if you can’t finish it beforehand, don’t worry. We’ll gladly provide the required forms and guide you through them upon your arrival.

NEW PATIENT FORMS
When You Come to the Office
Our new patient onboarding process spans two visits. On your first day, we’ll conduct a consultation, examination, and X-rays if needed. You’ll also receive your initial adjustment, designed to provide immediate relief.
The second day of your visit involves reviewing your X-ray findings and discussing your customized treatment plan. You’ll receive another adjustment, and we’ll outline any further care you may require.
Financial Responsibility
During your first visit, we’ll discuss the initial exam’s cost and what it entails. Our friendly front desk staff, including our office manager and chiropractic assistants, will guide you through this process. On your second visit, we’ll delve into the total cost of your tailored care plan.
We accept insurance assignments for motor vehicle accidents and personal injury, including third-party and attorney-managed cases. Medicare and TriWest are also welcome here. You can use your health savings account (HSA) for our services.
